EPIC: Phase-3 cleanup (LAST)

Umbrella for the Phase-3 cleanup/deletion work — runs LAST, after go-live, per the sequenced roadmap (FIX → VERIFY → GO-LIVE → CLEANUP). Dead-code deletion uses DI-aware triple-V&V; doc/side-file moves wait on issue-coverage confirmation; the stale-branch reconcile + repo-structure changes are owner-signed-off.
