Remove CodeQL and Codacy integrations (#397)

- Removed .codacy.yml configuration file
- Removed .github/workflows/codacy.yml workflow
- Removed .github/workflows/codeql-analysis.yml workflow
- Removed CodeQL and Codacy badges from README

These were failing due to authentication issues.
