[BUG]: Incorrect logical name shown for field

Description

I have a Contact form called Primary Participant that uses a custom field for the last name called Participant Last Name. When using the Level Up 4.1.3 to show logical field names, the logical name shown for Participant Last Name is incorrectly shown as lastname instead of the correct ci_participantlastname (screenshot 1). However, using All Fields (screenshot 2) and the form editor (screenshot 3), the value is clearly stored in ci_participantlastname.

Steps to Reproduce (for bugs)

1. Open an existing Contact or create a new one using the Primary Participant form
2. Show logical names with Level Up 4.1.3
3. Observe the logical name shown for Participant Last Name is lastname

Expected Behavior

Logical name shown for Participant Last Name should be ci_participantlastname
